Odometer Disclosure Statement

The Odometer Disclosure Statement and Statement of Prior Use is used by motor vehicle dealers in conjunction with non-conforming Certificates of Title when acquiring a vehicle by trade or purchase from a private individual or corporation. It could also be used in acquiring a vehicle from, or selling to, a licensed dealer if the reassignment of ownership is completed on the non-conforming title. (The MV2115 would be more commonly used.) Part A and B are completed when the vehicle is sold. Part C is completed when the vehicle is resold at retail. Part B odometer disclosure statement can be used only with non-conforming titles. The Odometer Disclosure Statement is a three part carbonless form available blank or imprinted in black ink.
