The Odometer Disclosure Statement (Leased Vehicle) and Statement of Prior Use (2C) is for use by Wisconsin Lessors at the end of a lease. This form is used by the lessee to make disclosures when the leased vehicle is surrendered, regardless of the date the lease was initiated.
