Request for Lien Release, 100/pkg

In Wisconsin, lending institutions have thirty days to send out a lien release unless they receive a written request. Upon receipt of a written request, lending institutions are required by statute to send the lien release within ten days. The Request for Lien Release form provides a single document that informs the lending institution of the parties involved, requests that the lien release be sent to the dealer and requests that it be sent within the statutorily defined ten day period. Both the dealer and the title-holder of the trade-in must sign the form. The request for Lien Release form is 2-part carbonless with a top stub. Size: 8 1/2" x 11 3/4".
