Odometer Disclosure Statement

The Odometer Disclosure Statement and Statement of Prior Use is used by motor vehicle dealers in conjunction with non-conforming Certificates of Title when acquiring a vehicle by trade or purchase from a private individual or corporation. This form is also used to capture vehicle history and use information from sellers.
