Odometer Disc Stmt (Leased Vehicle) and Stmt of Prior Use at End of Lease

The Odometer Disclosure Statement (Leased Vehicle) and Statement of Prior Use (2C) is for use by Wisconsin Lessors at the end of a lease. This form is used by the lessee to make disclosures when the leased vehicle is surrendered, regardless of the date the lease was initiated.
