Special Parts Order Form. 4-part

This special parts order form has a unique design that provides a gummed back label from part 3 of the form and a two part postcard that serves as a postcard notification and a second and final notice that the part's have arrived and are ready to be picked up or installed. Printed in red and blue ink, the red ink highlights the critical information and the special "Second Notice" language on the postcard. This form may be ordered stock as shown or imprinted and numbered to your specifications. Paper sequence is ply 1- white 15#, ply 2 - white 15#, ply 3 - white 33# gummed back, ply 4 - white card stock.
